Category title in results?

Hello, we are looking for a WooCommerce filter plugin that allows us to filter products by 1) Tag 2) Category and 3) Attribute. It appears the Super Speedy Filters plugin can do that, so that's great. We also need it to display results across those 3 filters by category with category heading and have some control over how the results are displayed. Is this possible? I will add a screenshot of the mockup we are trying to achieve in the "Private Information" below. Thank you.

Yes, it can filter all 3 of those things at once – you can filter any combination of stuff really and the URLs will be prettier than any other plugin, e.g. /product-category/dispatch/field-training/modern-crisis/product-tag/dispatch/state/california.
Regarding changing the appearance of the results – that is up to you, we don’t touch that, but it’s a fairly simple matter of editing your archive-product.php template to achieve the look you want.
